Pool keys: If any card is lost or stolen, a replacement is available for $25.00 payable to Oak Forest Village HOA. Please follow this link to order a replacement amenity key. Please note that only 1 key is permitted to be activated per household.

The undersigned lawful property owner in Oak Forest Village does hereby submit this proxy to be used for quorum purposes and appoints: or if left blank, the President, as my proxy to act on my behalf and to cast all votes which the undersigned is entitled to cast at the Annual Meeting and at any and all adjournments thereof with full power of substitution, as follows.
